Arteriosclerosis cerebri. Autopsy examination of 79-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, alercen blue, PAS and van Gieson method.In the left frontal lobe, a foci of hemorrhagic malacia was found destroying the cortex and partially the white matter of the middle and lower gyrus. Numerous perivascular and loose leukocytes were seen in the cortex. Many perivascular circular hemorrhages and extravasations through the diapedesis were noted. The tissue structure was significantly loosened. In these areas nerve cells loss was observed. The vessels were edematous, the endothelium was dilated, and multiple vacuolations and ischemic type disorders were seen in the border of the focus. A focus of diffuse necrosis was seen in the white matter. Numerous macrophages, stimulated vessels, gliosis, and multiple tissue defects were visible. The vessels were fibrotic and partially vitrified. In the meninges numerous macrophages with hemosiderin, histiocytes and a number of partially hemolysed blood cells were present. In the right ganglion, an old focus of malacia was visible. In the central part of the focus there was a cavity inside of which macrophages with hemosiderin, remnants of decaying tissue, vessels and collagen fibers were visible.
