Arteriosclerosis et atheromatosis vasorum meningealium precipue in regione fissae Sylvii. Autopsy examination of 68-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and Van Gieson's method.In the inferior frontal gyrus, insula, hippocampus, occiput and cerebellum, extensive foci of hemorrhagic malacia were found.The localization of all lesions was cortical-subcortical, in some areas cortical malacia with numerous small hemorrhages in its area predominated, in others particularly subcortical white matter was disintegrated.In fields of malacia, leukocytic infiltration and symptomatic perivascular reaction was observed. In several subcortical fields foci of tissue thinning were seen, which gave the impression of incomplete necrosis, older than fields of fresh malacia. The vessel walls were very markedly thickened and the lumen irregularly narrowed.
