Haemorrhagiae punctatae recentes corticis occipitalis. Arteriosclerosis cerebri gradu maiore. Autopsy examination of 56-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and van Gieson method.The focus of disintegration in the basal nuclei contained numerous macrophages and numerous rod-shaped microglia cells. Capillaries and a network of fibroblasts were infiltrating the focus. Single leukocytes were observed. Towards the periphery, hypertrophied astrocytes were seen. Numerous discrete symptomatic lymphatic infiltrates were seen around the vessels. The walls of capillaries and arterioles were markedly thickened with fibrous and vitreous lesions. Small fresh hemorrhages were found in the occipital cortex. Nerve cells were mostly preserved, but some showed a significant damage. Few scattered leukocytes were visible. The vessels also had small lymphocytic infiltrates. The soft meninges adjacent to the cortex were thickened. Many macrophages were present in the lumen of the vessels in the meninges. In the cerebellum, atrophy of the granular layer of the cortex was observed in several areas. These areas were filled with hypertrophied Bergman's glia. In the Purkinje cell layer, defects and cellular disorders of various types were present. Hemorrhages were found in the granular layer, often separating this layer from the Purkinje cell layer.
