Abstract:

Histological diagnosis: Meningoencephalitis versimilliter. Autopsy examination of 90-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with H-E, Heidenhain and Van Gieson's method.Inflammatory process was found in the meninges, passing through the vessels and through the fluid pathway into the parenchyma, presumably superimposed on much older atherosclerotic lesions. The inflammation was lymphocytic-macrophage type and with the presence of giant and epithelioid plasma cells. Inflammatory nodules in the ependyma suggested specific inflammatory changes - tuberculosis was suspected. Parenchymal changes were located mainly in the white matter, damaged in a diffuse manner - glial scarring, thinning of the tissue, spongy state, isomorphic gliosis, large perivascular lacunae were seen.Cellular defects (including Purkinje cells) with marked gliosis were observed in the cortex. An atherosclerotic process was observed in vessels of all calibers.Histological diagnosis: Meningoencephalitis verisimiliter tbc. in casu cum encephalopathia arteriosclerotica.
