Abstract:

Histological diagnosis: Ischemia diffusa hemisphaeri dextri cerebri. Hyperaamia et oedema cerebri. Atrophia cerebri simplex. Arteriosclerosis gradu mediocri, arteriosclerosis gradu maioris. Autopsy examination of 77-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, Heidenhain and Van Gieson's method.In specimens from the right cerebral hemisphere, the predominant change were spongy changes of the superficial layers of the central cortex and insula and gliosis of the white matter of the entire hemisphere. Within the basal nuclei diffuse gliosis, stimulated microglia and increased gliosis were seen, as well as areas of vacuolation and moderate hypertrophy with preserved layering. Many nerve cells, both in the cortex and pons contained lipofusin deposits. Features of edema and congestion were observed throughout the brain. There were no macrophage reaction anywhere. Numerous vitreous changes of arterioles were found, sometimes leading to the closure of their lumen. The whole picture suggests acute ischemia within the right middle cerebral artery superimposed on chronic vascular changes..
