Histological diagnosis: Foci haemorrhagicae ad encephalomalacias in regione thalami et lobi occipitalis sinistri, Status lacunaris. Atrophia cerebri. Autopsy examination of 72-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, Heidenhain and Van Gieson's method.A hemorrhagic focus surrounded by a band of spongy-changed tissue with numerous macrophages scattered in the tissue and clustered around vessels was seen within the thalamus. The focus reached the ventricular wall. Similarly, spongy changes with macrophage and trichomes reaction were present around the hemorrhagic focus in the white matter of the occipital lobe. Numerous small postsoftening cavities were observed throughout the specimen of the basal nuclei. Extravasated blood was present in the ventricles and subarachnoid space. The soft meninges showed cellular and fibrous reaction. The cerebral cortex was markedly thinned, with obliteration of layering in some areas. The white matter of the cerebral hemispheres showed diffuse gliosis and small areas of softening. In the cerebellum small foci of softening, as well as loss of Purkinje cells and Bergman glial cells were found. Prominent features of edema and congestion and perivascular infiltration throughout the brain were observed. Many arteries had atherosclerotic plaques narrowing the lumen. Quite a few arterioles showed intimal edema with wall dissection. Most of the arterioles were vitreously altered., oedem et hyperaemia cerebri. Arteric- et arteriolo-sclerosis gradu maiori
