Clinical, anatomical and histological diagnosis
Histological diagnosis: Encephalomalacie inveterata multifocalis. Status lacunaris. Arteriosclerosis gradu majoris. Autopsy examination of 66-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with H-E and Van Gieson's method.The edge of an old encephalomalacia focus was noted in the occipital lobe. Glial scar formation was seen in the white matter and partially in the cortex. Numerous gemistocytes and astrocytes as well as single macrophages were observed. The number of cellular elements was reduced. Lymphocyte-like cells were present at fibrillar vessels. In the cortex, foci of incomplete necrosis were seen, besides cellular atrophy. Two developing large perivascular lacunae were found in the basal ganglia. The cells showed a significant degree of fatty degeneration. In addition, smaller perivascular necroses were present. The small vessels were vitreously altered. In the globus pallidus, vessels with pseudocalcium salts were found, larger vessels presented with fibrotic and atheromatous changes. Features of congestion and slight edema were observed at all sites
