Description:

Clinical, anatomical and histological diagnosis

Abstract:

Histological diagnosis: Foci encephalomalaciae /recens et inveterata/. Arteriosclerosis. Autopsy examination of 61-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with H-E and Van Gieson's method. In the left temporal lobe there was an old focus of malacia destroying white matter and partially the cortex of the superior and middle ganglion. In the central part of the focus, a cavity was visible, in the edges of which remnants of brain tissue and a large glial and mesodermal reaction were found. In the cortex of the injured ganglia, the molecular layer was preserved only in parts; necrosis in the dissection phase was observed in other parts. In the border of the focus, the tissue structure was loosened. In the cortex, large cellular depletions and ischemic disorders were observed. In the right parietal lobe, fresh hemorrhagic necrosis destroying the cortex over a large area was present; circular, fissure, perivascular hemorrhages as well as stimulation of vessels and glia were observed. Besides, multiple foci of perivascular spongiosis were seen in the cortex. The whole brain showed features of edema and congestion. The vessels were vitreous-fibrously altered.
