Histological diagnosis: Malacia e compresione medullae spinalis.Autopsy examination of 70-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and van Gieson method. At the level of the lesion, the medullary tissue dissolved converting to a loose, reticular, multi-eyed composite. The neurons had homogenized. In the loose "eyes" of the reticular tissue, clotted clusters of proteinaceous transudate were visible, staining homogenously eosinophilic. The posterior radicles were swollen. At this level, the vessels had very thick and vitreously altered walls. At the superior thoracic level there was marked swelling of the radicles. The neurons were better preserved. The lumbar spinal cord was well preserved, with numerous neurons quite severely steatotic. The midbrain without major changes. Segmented Purkinje cell prolapses were found in the cerebellum. Single, perivascular, plaque-like desolations were observed in the granular layer of the cerebellum.
