Description:

Clinical, anatomical and histological diagnosis

Abstract:

Histological diagnosis: Encephalitis lymphocytaria acuta.Autopsy examination of 14-month-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and Heidenhain method. Heidenhain staining showed small perivascular foci of myelin fading in the white matter of the hemispheres, in the cerebellum, and in the brainstem. These foci at high magnification corresponded to fields of dissection and fragmentation of myelin sheaths. Predominantly in the white matter of the hemispheres, and to a lower extent in the brainstem and cerebellum, numerous vessels with discrete lymphocytic infiltrates were observed. The astrocytic glia were stimulated, concentrated. Several more severely infiltrated vessels were seen in the basal ganglia, thalamus. Patches of proliferating microglia were seen near the ventricular system. Cortical capillaries were stimulated, often with small infiltrates - loose lymphocytic infiltrates in the meninges. Significant diffuse ischemic changes were observed in the thalamus and altered Purkinje cells and dentate nucleus in the cerebellum.
