Abstract:

The author presents a case of fluid dissemination of multiple myeloma within the limits of the central nervous system, not yet met with in literature. Prior to dealing with this case the author describes in detail the macroscopic as well as microscopic morphological picture in Kahlen's disease and discusses the localizations of multiple myeloma which are usually the cause of neurological com¬plications. In accordance with data contained in literaturę he stresses the rarity of direct infiltration of the tumour into the central nervous system either as metastases or in the form of a continuous growth. In this latter case a special protective role is played by the dura mater which constitutes a sui generis „barrier“ impeding the penetration of the tumour. The effect of the tumour on the central nervous system is usually reduced to mechanical pressure and to chemical and toxic processes. In the described case—presented in detail from a clinical point of view by M.Strumień —a single myeloid tumour penetrated in the region of the clivus Blumenbachi into the dura mater of the base of the skull and infiltrated subse¬quently the soft meninges and the parenchyma of the brain stem within the pons. Near the parenchymal Focus forming a direct passage of the osteal tumour to the tissues of the central nervous system the author noticed accumulations of tumourous cells in the soft meninges of other parts of the brain stem and in the area of pons Varoli. This localization of secondary changes as well as the vascular infiltration of the tumour in to the parenchyma of nervous tissues are, according to the author, an expression of a fluid dissemination of the tumour in the central nervous system. Vascular changes (hyalinization), hemorrhagic changes and pictures of proliferation of glia and cells of Schwann's sheath in sections adjacent to the tumour are regar¬ded by the author as an evidence of the toxic action of the tumour.
