The authors report clinical and morphological observations concerning 19 cases of primary intracerebral sarcoma.Young age of patients, short course of disease before the operation, relatively good general state of patients, lack of neoplastic intoxication inspite of severe neurologic disorders in a majority of patients, and good effects of surgical treatment are the outstanding clinical features. The authors emphasize the high sedimentation rate of red blood cells in all the patients. They point out that primary cerebral sarcomas occur more often then was formerly thought.Prominent cellular polymorphism, pronounced anaplastic features and very close relationship of neoplastic cells to blood vessels in all the tumors regardless of the type of their morphology and of the caliber of blood vessels, are the characteristic features.The authors assume that primary cerebral sarcomas originate from connective tissue cells of the cerebral blood vessels. It seems impossible to determine morphologically from which elements of the vascular wall (adventitial or cells of meningeal sheaths of intracerebral blood vessels) the tumors originate. The morphology of the tumors does not reflect their histogenesis. An explanation of the histogenesis of the tumors should be sought rather in histochemistry and in observations on tissue culture.Pluripotentiality of the original maternal tumor cell, regressive changes in the neoplastic tissue and cellular developmental malformations are responsible for cellular polymorphism and varieties of sarcomas.The authors emphasize the relative value of classifying sarcomas into groups of primary cerebral tumors and tumors of meningeal origin. The example of intracerebral arachnoidal sarcoma is cited.Attention should be given to the lack of reticulin fibers in certain sarcomas, in whole or in large parts of the tumor. Staining of reticulin fibers although very useful in the diagnosis of sarcomas, should be considered in conjunction with the positive and negative morphologic features, giving full consideration to the clinical data. The data of other authors, as well as personal observations, indicate that the postoperative course in patients with cerebral sarcoma is better than in those with glioblastoma multiforme or with sarcoma outside the central nervous system.Familiarity with the morphologic differential diagnosis of sarcomas and glioblastomas multiforme during intraoperative examination is of great importance to the surgeon in deciding for or against the operation.
