Cicatrix in regione gyri frontalis medis. Oedema et hyperaemia cerebri. Autopsy examination of 55-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with hematoxylin-eosin, PAS and Van Gieson's method.In the left hemisphere, an extensive foci of encephalomalacia destroying the cortex and white matter of the superior and middle frontal cingulate cortex was found .The focus was partially in the stage of demolition and partially in the stage of glial-mesodermal organization. Cavities had formed in the cortex, in which remnants of neural tissue, macrophages, vessels, and a proliferation of connective tissue fibers were visible. In the white matter, loosened tissue, focally spongy in structure, with reduced number of cellular elements was observed. In some places macrophages and stimulated vessels were found. The cortex showed numerous cellular defects, locally incomplete necrosis. The remaining nerve cells showed features of chronic and ischemic disease. The vessels were maximally filled with blood, with symptomatic inflammatory reaction in the vicinity of some. The tissue-vascular borderline was loosened, some vessels were filled with leukocytes. Van Gieson's staining showed vitreous-fibrous type of vessel wall changes.
