Abstract:

Histological diagnosis: Meningo-encephalitis purulenta recurrens.Recurrent purulent encephalitis.Autopsy examination of 8-month-old infant was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, Cresyl violet, Heidenhain, reticulin and Van Gieson's stain.Analyses of all the specimens taken revealed an extensive inflammatory process in the meninges, brain tissue and subcrepitally. In the meninges, the leucocyte infiltrate was almost completely resorbed and granulomatous organization was visible. Small fields of necrotic tissue surrounded by a histiocytic and collagenous capsule, often containing giant multinucleated cells, were observed. Vitreous changes were observed in vessels. Single leukocytes, mainly histiocytes and macrophages, were adjacent to the vessels. Corresponding to the vascular changes, neuronal vacuolation and necrosis in the white matter were present. An old inflammatory process bearing clear features of healing by scarring formation and granulomatous reaction was found around the ventricular system. Fresh purulent exudate was found, with numerous leukocytes arranged in clumps adhered to the ventricular walls, mainly in the lumen of the lower part of the ventricular system.
