Abstract:

Histological diagnosis: Aneurysm arteriae cerebri anterioris sinistri. Haemorrhagia cerebri recens. Autopsy examination of 63-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and Van Gieson's method.A large aneurysm was seen on the medial surface of the left frontal lobe simple ganglion, corresponding macroscopically to the described nodule in the left anterior cerebral artery. The abnormal vessel had a wall of varying thickness; highly altered, with a glassy-fibrous structure. The layering of the vessel structure was disturbed. The main components of the wall were collagen fibers, muscle fibers, and fibroblasts, intertwined with each other. In the area of the greatest wall destruction, the vessel was damaged and blood leaked into the parenchyma destroying the cortex and white matter of the frontal lobe as well as the simplex cortex and superior frontal ganglion. Besides, numerous macrophages with hemosiderin and histiocytic-like cells were observed around this vessel. The hemorrhage was fresh and intense leukocytic infiltration was seen in areas. The cortex showed vacuolation and ischemic disorders. Whole brain revealed features of edema and congestion. The vessels were fibrously altered. Blood was present in the subarachnoid space of the cortex and in the ventricular system
