Clinical, anatomical and histological diagnosis
Histological diagnosis: Encephalomalacia recens hemisphaerii sinistri cerebri in decursu thrombosis arteriae cerebri mediae. Status lacunaris. Arteriosclerosis gradu magno. Arteriosclerosis gradu mediocri. Autopsy examination of 75-year-patient was performed. Neuropathological evaluation in light microscopy was based on routine gerontological staining panel of brain paraffin sections.A left frontal cortex specimen revealed a focus of tissue disintegration surrounded by an area of fresh malacia with capillaries, a blood-derived infiltrate and single macrophages. The lesion involved both the cortex and the white matter. In the brain stem, an old post-malacia cavity with extravasated red blood cells was present on one side. The basal nuclei of the preserved hemisphere showed status lacunaris. A field of spongiosis was present in right frontal cortex. Thinning of neural cellular elements was seen in other areas of the cortex. Numerous perivascular sinuses were present in the white matter of the cerebral hemispheres and cerebellum. The granular layer of the cerebellum was markedly thinned. The soft meninges showed cellular and fibrous reaction. Single extravasated blood cells were seen in the subarachnoid space over the cerebral hemispheres. Large caliber vessels like basal and vertebral vessels showed atherosclerotic type IV lesions, smaller cerebral arteries showed unremarkable vitreous lesions, while vessels in the sinuses of the basal nuclei were thin-walled, atrophic.
