Clinical, anatomical and histological diagnosis
Histological diagnosis: Encephalomalacia inveterata et recens.Autopsy examination of 69-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and van Gieson.In the left parietal lobe, there was an extensive focus of malacia destroying the white matter and cortex. Inside and at the edge of the focus there were not numerous clusters of macrophages. The focus of malacia was surrounded by stimulated astrocytes, besides, the edge of the focus consisted of numerous young vessels. Around the vessels located slightly further at the periphery of the focus, single- to double-row small round cells infiltrates and single macrophages were present. A focus of fresh necrosis was seen in the right cerebral hemisphere. The few preserved nerve cells in this area were characterized by highly eosinophilic protoplasm. The vessels were markedly dilated and filled with leukocytes. Swelling of the nerve tissue was evident on all specimens. The nerve cells of the ammonal cortex, parietal lobe, and temporal lobe showed features of steatosis. The vessels of the meninges and cortex from the aforementioned areas were dilated and filled with blood. The vessels generally presented features of moderate fibrosis. Putrefaction.
