Abstract:

Histological diagnosis: Encephalopatia portosystemica.Autopsy examination of 69-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and Heidenhain method.Profound pathological changes were found in the cerebral cortex, consisting of diffuse loss of nerve cells, mainly in its deeper layers. These defects were most prominent in frontal and central cortex /central frontal ganglia/. In the central cortex, the cellular depletions were accompanied by the formation of small cavities, which merged together and gave an image of a spongy state. The cellular defects were not accompanied by glial reaction. The nerve fibers in the vicinity of the cavities had normal myelin sheaths. Numerous small reaction-free cavities were also seen in the striatum and globus pallidus. Some of these lay adjacent to vessels, and some had a look-alike appearance in the central cortex. Nerve cell steatosis was significant in some areas. The vessels showed slight fibrosis of the walls.
