Description:

Clinical, anatomical and histological diagnosis

Abstract:

Histological diagnosis: Polymicrogyria spura. Hydracephalus internus. Encephalopathia anoxica.Autopsy examination of 10-month-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, cresyl-violet and Heidenhain method. In the frontal, temporal and occipital lobes, single accessory gyri were found, but with normal microscopic structure, with the proper number of layers. Significant internal hydrocephalus caused compression of brain tissue, distortion of the contours of the basal ganglia and thinning of the white matter layer. However, the cellular structure of the basal ganglia was normal. Along the entire cortex, a layer of spongy white matter laid on the "u" line of the fibers. Thinning of white matter was probably caused by circulation disturbances /stasis or hypoxia/ at the border between cortical and subcortical vascularization. The cortex showed numerous cell desolations and very numerous ischemically altered neurons. The structure of the brainstem was normal. Along the line of lining in the occipital lobe, a high density of glia with fetal characteristics was observed. In the white matter, the glia was generally highly concentrated. There was a slight increase in the number of lymphocytes and endothelial cells in the meninges and a fairly significant congestion of medium caliber vessels.
