Description:

Clinical, anatomical and histological diagnosis

Abstract:

Histological diagnosis: Meningo-encephalitis septica incipiens. Haemorrhagiae punctatae multiplices. Hyperaemia cerebri generalisata. Autopsy examination of 30-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain frozen sections stained with Hematoxylin-eosin.Meningeal specimens on the convexities and grooves of the cerebellum showed a multiplication of lymphocytic cells, epithelioid cells, and single leukocytes. Accumulations of leukocytes were seen in the vessels at the level of the pons. In the hypothalamic region, in the nucleus supraopticus, below the caput nuclei caudati, astrocytic proliferation and in places microglia were visible. The image was dominated by enormous congestion of the entire vascular network, reaching down to the finest cortical capillaries. In medium and large vessels, hemorrhages and perivascular hemorrhages were seen, often with blood cells mixed with leukocytes in the perivascular spaces. In addition to the obvious signs of vascular barrier disruption, intra-tissue edema was less pronounced. The parenchymal damage was relatively minor: in the basal ganglia, in the thalamus, then in the nucleus supraopticus, in the corpus geniculatum mediale, in the olives and in the nucleus dentatus very numerous steatoses were present.
