Description:

Clinical, anatomical and histological diagnosis

Abstract:

Histological diagnosis: Encephalitis lymphocytaria.Autopsy examination of 2-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin.In the scrapings from the anterior parts of the hemispheres: from the frontal region and from the basal ganglia, we found a fairly uniform density of glia in the white matter and discrete but undoubted lymphocytic infiltrates of several to a dozen elements. These infiltrates in the frontal region accompanied almost all vessels. Towards the occipital and stem, signs of inflammation weakened, the vessels had stimulated endothelium and were mostly filled with blood cells. The tissue showed marked features of edema and widening of perivascular spaces. Prominent atrophy of cortical cells (especially in the occipital region) was observed, affecting mainly layers III and V. There were many ischemic cells among the remaining cells. On the midbrain slice, substantia nigra cells and oculomotor nerve nuclei were visible - both of them without changes, except for swelling of the underlying substrate. The overall picture is consistent with cerebral lymphocytic inflammation with a vascular component.
