Clinical, anatomical and histological diagnosis
Histological diagnosis: Focus encephalomalaciae recentis hemisphaerii dextri cum focis haemorrhagicis secundariis in regione pontis. Embolia arteriae cerebri mediae, dextrae. Atherosclerosis. Autopsy examination of 64-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and van Gieson method.In the parietal area of the right hemisphere, a very fresh foci of malacia with a thinned tissue structure, vascular congestion and small hemorrhages were found. Above the focus, meningeal lesions were seen in the form of vascular congestion, small hemorrhages, and single lymphocytes in the periphery. Cellular loss were noted in the cortex. Numerous secondary hemorrhages were observed in the pons, destroying the nuclei of this area. As in the cortex, thinning of the pons tissue structure was observed. The right middle cerebral artery was filled with a material composed mainly of clotted blood, amorphous masses, lymphocytes, neutrophilic leukocytes and a large number of acidophilic ones. Adherent thrombi were present. The vessel wall was atheromatically altered with plaques, stratified intimal elastic membrane, and thinned media layer.
