Clinical, anatomical and histological diagnosis

Abstract:

Histological diagnosis: Haemorrhagia cerebelli recens. Haemooephalus int. Arteriosclerosis gradu magno /atheromatosis, arteriolosclerosis. Autopsy examination of 80-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, van Gieson, Griedleys and Heidenhain method. In the white matter of the cerebellum, a fresh hemorrhagic focus was found, locally extending to the granular layer and partially occupying also the cortex. In the central part of the focus in the cerebellum, the blood was completely hemolyzed, and clusters of leukocytes were seen in some areas. At the periphery of the focus, masses of blood cells and disintegrating leukocytes were visible. No glial reaction was observed within the surrounding tissue. Purkinje cells were thinned and homogenized. In one of the cerebellar lobes, small foci of necrosis were seen, involving the fine and granular layers, extending into the white matter. Numerous collagen fibers were present within the focus. In both the basal ganglia and cerebellum, increased atherosclerotic changes were observed, primarily vitreous and fibrous changes. Within the globus pallidus, pseudocalcium deposits were present in the wall of single vessels. Small symptomatic infiltrates and macrophages with hematoidin were seen around single vessels. Nerve cells presented fatty degeneration.
