Clinical, anatomical and histological diagnosis
Histological diagnosis: Encephalomalacia alba partim haemorrhagica recens.Autopsy examination of 82-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and van Gieson method.A right parietal region specimen showed a necrotic focus involving both the cortex and the white matter. The cortex was almost completely devoid of nerve cells. The preserved nerve cells showed increased retrograde changes of ischemic type. The greatest number of preserved nerve cells was found in layer II and in layer V. Glial cells were maintained. The population of glial cells in the white matter was preserved. The structure of the cortex and white matter was loosened, with a picture of spongy lesions in places. The vessels mostly showed obliteration of their normal structure and significant endothelial stimulation. Perivascular dense clusters of leukocytes were observed. Leukocytes were also seen lying freely in the tissue. Some sections of the cortex showed very numerous perivascular hemorrhages by diapedesis, comprising the typical picture of hemorrhagic necrosis. The soft meninges showed leukocytic infiltrates and red cell masses.
