Clinical, anatomical and histological diagnosis

Histological diagnosis: Atrophia cerebri simplex, status lacunaris. Arterio et arteriosclerosis gradu majoris. Oedema et hyperaemia cerebri. Autopsy examination of 89-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with H-E, cresyl-violet, orcein, Heidenhain, Holzer and Van Gieson's method. In specimens from the cortex of the cerebral hemispheres, a pronounced loss of nerve cells was observed. The layering was preserved. Large areas of white matter spongiosis were present in all regions of the brain. In the parietal region the spongiosis also involved the cortex. Diffuse fibrous gliosis was seen in the cortex. In the wide subarachnoid space, reticular fibers and single extravasated blood cells were present. Many small cavities surrounded by fibrous gliosis were found in the basal nuclei, some of which contained lymphocytes and macrophages. Hemosiderin deposits were present throughout the specimen. Prominent subependymal gliosis was evident. Numerous areas of spongiosis were found in the trunk, both in the midbrain and in the bridge. The above changes were prominent especially within the substancia nigra, some of the cells had disintegrated. In the cerebellum, diffuse thinning of the granular layer was seen. In the vessels, multiple vitreous changes of the arterioles were predominant. In medium caliber arteries, vitrification of the middle layer and fragmentation and dissection of the inner elastic membrane were occasionally observed. Features of congestion and edema were found throughout the brain.
