Clinical, anatomical and histological diagnosis
Histological diagnosis: Encephalomalacia recens et inveterata. Autopsy examination of 75-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with H-E, PAS and Van Gieson's method. An old focus of malacia in the stage of advanced glial-mesodermal organization, destroying mainly the cortex, was found in the fusiform gyrus and inferior temporal gyrus. A fresh focus of malacia was present in the left frontal lobe in the region of middle cerebral artery vascularization. The tissue in the focus, characterized by worse staining, was loosened, spongy in places, with decreased number of cells. In several parts of the cortex leucocytic infiltration was visible, in addition in the cortex ischemic disorders appeared. Remaining nerve cells presented with fatty degeneration. Purkinje cell atrophy was observed in the cerebellum. PAS positive material was found in stimulated and hypertrophied astroglia, macrophages, vitrified vessels and fatty degenerated nerve cells. There was inflammatory reaction of lymphocytic cells in the meninges. Large atherosclerotic lesions, features of edema and congestion were seen throughout the nervous system..
