Abstract:

Histological diagnosis: Status lacunaris.Autopsy examination of 67-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, van Gieson, Heidenhain and Holzer method. Frozen sections were stained with Bielschowsky method.The brain presented a picture of an extremely advanced and generalized into numerous structures status lacunaris. In the putamen, near the medium vessels, there were typical large sinuses. In the nucleus caudatus and thalamus, there was thinning of tissue around the capillaries. All vessels of the semiovale center were surrounded by spongy disintegration fields. Similar numerous sinuses were present in hippocampus, in the pons, and in the medulla oblongata. The cerebellum was relatively better preserved. Most of the vessels located in the sinuses were characterized by significant proliferation of collagen and reticulin. A very rich proliferation of young capillaries was visible between the vessels. Around the sinuses and at the vessel walls, surrounded by spongy necrosis, fibrous gliosis was observed. A large apoplectic cavity filled with loose connective tissue was present at the border of the thalamus and capsula interna. Very extensive cellular loss was observed in the cortex, with only very sparsely scattered neurons visible in places, with no stratification.
