Clinical, anatomical and histological diagnosis
Histological diagnosis: Focus encephalomalaciae hemispherii dex. in stadio desintegrationis. Arteriosclerosis gradu mediocri. Autopsy examination of 72-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin and van Gieson.The specimens from the frontal gyri showed extensive fading, mainly of the white matter, corresponding to a large area of fresh necrosis. In the central part of this focus, single glial cells, few vessels, and perivascularly macrophages and leukocytes were visible. More peripherally to the focus, at the border of the cortex there was a multiplication of capillary vessels, filled with blood, with walls infiltrated with leukocytes and macrophages. In the cortex, in addition to the described changes, small foci of complete necrosis were visible.I n the soft meninges adjacent to the focus, significant leukocyte-macrophage infiltration was found. On specimens from the basal ganglia, similar changes were seen. Compared with the lesions in the white matter and cortex, however, the necrotic lesions were larger, the tissue more infiltrated with leukocytes. The vessels were filled with leukocytes. Nerve cells in the cortex, even outside the focus were ischemically altered. In the entire nervous system, almost all nerve cells were markedly steatotic. Most capillaries and arterioles showed atherosclerotic changes in the form of vitreous and fibrous degeneration.
