Abstrakt:

The author considers a case of the organic disease of the spinal cord in the form of paraplegic relatively rare pathogenic mechanism with vasculomotor disorders developed on the basis of lesions injuring the medulla. The author proceeds to discuss the case by a brief review of the literature on the role of vascular changes in pathogenesis cf the non-inflammatory organic disorders of the medulla. The case concerns a 56 year old woman, in whom after some weeks following a pseudo-influenza infection, a subacute progressive syndrome of incomplete diagonal injury of the medulla with simultaneous psychic changes developed. Clinically an organic medullar injury was diagnosed, vasci'lating between disseminated encephalomyelitis and metastasis of the neoplasmaof an unknown origin. The autopsy revealed a medullar malacia on the levelcf D3. A micrdscopic examination disclosed cerebral ischemia, disseminatedcortical degeneration in the form of the ischemic disease of nervous cells, medullar malacia with a secondary degeneration of pyramidal tracts and with hemorrhagic purpura of the spinal pia mater. Microscopically one was struck by the parenchymatous lesions of a vascular- origin character with relatively little advanced organic lesions cf the vessels. On the ba'sis of the correlation of these facts the vasculomotor disorders must be accepted as a basic factor in the developments of the described syndrome. Cortical lesions of a character of the disseminated ischemic degeneration of the nervous cells suffice to explain the described psychotic symptoms.
