Clinical, anatomical and histological diagnosis
Histological diagnosis: Foci encephalomalaciae praecipue corticalis regionis frontalis. Status lacunaris.Autopsy examination of 67-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained with Hematoxylin-eosin, van Gieson and Heidenhain method.In the frontal lobe, foci of malacia were found involving mainly the cerebral cortex. There were fields covered densely with macrophages, sometimes spongiosis were seen and numerous mast cells at the margins. In almost all foci an extensive connective tissue reaction was seen, mainly in the form of vascular proliferation. In the basal nuclei, tissue loss was found around some vessels, with spongiosis at the edges. The cortex was mostly characterized by very severe cellular desolations. Most of the vessels had thickened and fibrotic walls, less often endothelial edema was observed. Single round cells were observed perivascularly. Nerve cells were mostly ischemically altered, often steatotic. Features of edema were seen. Blood stasis was observed in the vessels of the thickened soft meninges.
