Clinical, anatomical and histological diagnosis
Histological diagnosis: Pachymeningosis haemorragica interna. Haemangioma durae matris. Arteriosclerosis cerebri. Venostasis cerebri. Malacia capitis nuclei caudatiAutopsy examination of 78-year-old patient was performed. Neuropathological evaluation in light microscopy was based on brain paraffin sections stained by H-E and Van Gieson's stain.The inner part of the dura was composed of sinusoidal capillaries, filled with blood and of extravasated blood in the wide spaces between the individual fiber bundles of the diffuse meninges. The choroidal arteries were characterised by thickened walls, while the veins had thin walls and enormous diameter ad were filled with blood. Stasis also affected the veins in the intercranial grooves. Macrophages were present in the meninges, often filled with hemosiderin. The compressed cortex showed neuronal thinning and widening of the vascular spaces. Generalized capillary fibrosis was observed in the brain parenchyma.Ventrally, glial scarring with the presence of macrophages was observed in the head of the caudate nucleus
